To Make An Appointment:

1. Contact the office where you wish to have an appointment. (See list of locations)
2. Describe the treatment you are seeking and referring physician, if applicable.
3. Request specific specialist or doctor, if applicable.

Prior To Your Appointment:

1. PRINT NEW PATIENT FORMS
   Please print, fill out the forms, and bring with you to your appointment.


2. If you cannot print the forms, let us know and the forms will be sent to you by mail.
   Please fill out the information and bring with you to your appointment.


3. Collect and bring with you to the appointment any previous test results, including labs,
   scans or x-rays. Bring any notes or other information your referring physician or primary
   physician feels we may need.


4. Bring your insurance card. If you have HMO insurance, please bring your referral also.

Day Of Appointment:

1. Please arrive 20 minutes before your scheduled appointment, if you are a new patient.
